When I saw that you could set up a title for pre-sale through Smashwords I started pouring through the KDP select rules and looking on the internet to see if I could offer the book for pre-sale and make it available to actually buy after the 90 day exclusivity ran out. I went to the website "Indies Unlimited" and found this article:
I was confident you could run them both at the same time as long as 1. I set the sample at only 10% and 2. that it was not available for sale (you could not buy a copy) until after the 90 days at KDP select ran out.
Well, now I can tell you DO NOT DO THIS!!!!
On June 10th (one day before Defenders completed its 90 days exclusivity) I got this email from Amazon titled your KDP select title:
We found the following book(s) you've published doesn’t meet the KDP Select content guidelines because it is available in digital format on another platform. Books enrolled in KDP Select must be exclusive to Amazon in digital format during the entirety of their enrollment in the program.

You may need to copy and paste the entire URL above into your web browser to see where we found your book(s). You can also do an online search for your book(s) to discover where else it may be available.
In order for your book(s) to remain in the KDP Select program, we'll need you to ensure that it is exclusive to Amazon within 5 days from the date of this email. If, after this 5-day period, your book(s) is still not exclusive to Amazon, it will remain for sale in the Kindle Store but will be removed from KDP Select. Upon its removal, your book(s) will no longer be eligible to earn a share of the KDP Select fund.
Please note that violations of the program’s exclusivity requirement may result in loss of KDP Select benefits, including participation in the Kindle Owners’ Lending Library (KOLL) and the usage of Kindle Countdown Deals or free promotion days. To ensure KDP Select benefits are not revoked, please be sure that all of your KDP Select-enrolled books are exclusive to Amazon in digital format at the time of enrollment.

Okay, so I sent a message back explaining that it was only a pre-order and not available for purchase.(At the time, if you followed the URL they gave me, there was a big button that said pre-order and under that it said this title will be available for purchase on June 13, 2014).
I got this reply to my explanation:
Thank you for your response and for taking the time to ensure that your books meet the requirements for KDP Select.
If your book is marketed in a way that indicates it is available for distribution, for sale or for free, then it is not eligible for KDP Select. For example, one of the possible indicators that the book is available for distribution may be the presence of instructions on another website or sales channel for downloading or obtaining the book, including by way of pre-order.
You may offer a sample, excerpt or teaser of your KDP Select-enrolled book on your website, as long as it doesn’t include a substantial portion of your book’s content. Up to about 10% of the book’s content is a reasonable amount. We strongly recommend you clearly indicate in the title of your sample that it is a sample, so that there won't be confusion regarding availability of your book on another sales channel.

Okay, I sent back an email explaining that it was the last (90th) day of my enrollment so it does not matter, it will be off KDP select in a few hours.
I got my Deposit slips for my European sales today and guess what THEY DEDUCTED ALL THE LENDING MONEY! In other words they shorted me all the KDP select funds that I earned back in April! I have sent them an email explaining all this and I will will keep you all posted.
The moral of the story: Do not put any titles up for pre-sale while you are enrolled in KDP select. Despite what I have seen on multiple websites, Amazon does NOT like it and WILL take your KDP select benefits from you for doing it.
